What Is Fertility Diagnostic Imaging?
Fertility diagnostic imaging plays a crucial role in evaluating reproductive health. Dr. Norbert Gleicher and Dr. David H. Barad utilize hysterosonograms (HSN) and hysterosalpingograms (HSG) to gain detailed insights into the uterus and fallopian tubes. HSN combines saline with high-resolution ultrasound to detect uterine abnormalities, offering a comfortable alternative to traditional methods, typically resulting in less cramping. However, it cannot replace hysterosalpingograms when it comes to evaluations of fallopian tubes. HSG is the standard x-ray study of the uterus and the fallopian tubes. It is designed to visualize the inside of a uterus (the uterine cavity). HSG also gives us a basic assessment of whether the fallopian tubes are unobstructed.

How Do HSN and HSG Work?
At the Center for Human Reproduction, Dr. Gleicher and Dr. Barad use fertility diagnostic imaging to provide detailed information. During an HSN, the patient lies down on a treatment table as if for a Pap smear, and the speculum will be inserted. Then, the provider will introduce a saline solution into the uterus through a flexible catheter. High-resolution ultrasound then captures clear images of the uterine cavity.
In an HSG, the procedure begins similarly, but instead of saline, a special dye that appears on x-rays is used. As the dye fills the uterus, x-rays are taken every few seconds. This reveals any abnormalities inside the uterus, such as a split cavity, uterine fibroids, or adhesions. As more dye flows into the uterus, some will begin to fill the fallopian tubes. If the tubes are unobstructed (patent) along their entire course, eventually the dye will "spill" out the other side. Digital images taken during hysterosalpingogram can be manipulated in a number of different ways to help a physician distinguish subtle abnormalities. Typically, 50 – 60 images are generated from a basic HSG.
